What would be sufficient medical documentation. Would the list of pills I have to take from an official health website count as corroborating medical documentation. I have pretty severe anxiety and my doctor has prescribed me anxiety medication.

Assuming this is a question about admissions, I would speak to the school directly, they'll tell you what to submit. Or at least provide better guidance. 

 

Having said that, I'd endeavour to submit a diagnosis if you have one.
