Does anyone know which schools ask for a generic reference letter where the referee just submits a letter and which schools ask for more than a letter such as asking the referee to fill out other paperwork? For example, Dalhousie asks for a secondary paper to be filled out. Does any other school require further documentation from a referee?

McGill has a form for referees to fill out, and for Ontario schools there is a similar form in OLSAS. These are the only ones I know about, but I imagine most of the others do this too. You can see the McGill one on their website if you are curious about specifics, the OLSAS one is similar.
